Heart Optimisation Clinic

We are pleased to announce the opening of our inaugural Heart Optimisation Clinic.

Located at our Unley Road Clinic, 313 Unley Road, Malvern the clinic will be conducted every Tuesday and
managed by heart failure specialists Dr Srini Srinivasan and Nurse Practitioner Ms Kath O'Toole.

Together they have implemented a patient centered approach to the assessment and management of patients with heart failure.

The Heart Optimisation Clinic will provide rapid access to clinic appointments, additional support for the care of complex patients and improved communication of assessment outcomes and management plans.

We believe that our Heart Optimisation Clinic will reduce patient admissions to hospital, improve functional status and quality of life for our patients.

sriniCardiologist: Dr G (Srini) Srinivasan

Srini graduated from the University of Madras, India in 1992 and gained experience in cardiology at the Madras Medical Mission, one of the premier institutes and tertiary referral centres for cardiac disorders in Chennai.

He completed his advanced cardiology training at the Greenlane Cardiovascular Centre and Middlemore Hospital in Auckland. While practising as a Consultant Cardiologist in New Zealand he was invited by the Cook Islands, Ministry of Health to lead a cardiac clinic in Rarotonga Hospital. Srini was awarded his FRACP in 2010.

To further advance his expertise in cardiac imaging Srini completed a Clinical Fellowship in cardiac magnetic resonance imaging at Flinders Medical Centre and is involved in clinical research projects in hypertrophic cardiomyopathy and heart failure with preserved ejection fraction.

Srini is a non-interventional Cardiologist with extensive experience in Echocardiography and Cardiac Magnetic Resonance imaging and has an interest in all aspects of clinical cardiology. In 2017 he accepted the central role in managing the inaugural Heart Optimisation Clinic at Adelaide Cardiology.

Srini consults at the St Andrew's Medical Centre, and our Leabrook, Unley Road, Gawler and Wallaroo Clinics.

 

kathNurse Practitioner: Kathryn O'Toole

Kath is a consultant cardiac Nurse Practitioner with extensive clinical experience in chronic cardiac care, secondary prevention and cardiac rehabilitation. Kath was authorised as a Nurse Practitioner in 2011 after completing her Masters of Nursing - Nurse Practitioner at the University of South Australia.

In 2012 Kath was awarded the SA Premiers Scholarship to support a Clinical Practice study in England. Following her study she was invited to Montreal, Canada to present a Clinical Practice Improvement project on ROSIER Stroke assessment tool in the Emergency Department.

She works closely with cardiologists at Adelaide Cardiology and provides an innovative cardiovascular prevention and rehabilitation service. By embracing a patient centered approach, Kath empowers her patients with the appropriate knowledge and strategies to assist them with managing their cardiovascular disease.

Kath is an integral member of the Heart Optimisation Clinic team and leads the nurse led Heart Failure Titration Management Clinic, focusing on optimisation of evidence-based pharmacotherapies, including a fexible diuretic regimen, patient/carer education, self-management strategies, lifestyle modifcations and psychosocial support.

Kath consults at the St Andrew's Medical Centre and our Unley Road and Modbury Clinics.
